The Blake’s memory care experts focus on the social, spiritual, cognitive, physical and emotional wellbeing of our residents.
We start by getting to know our residents.
Our certified dementia practitioners create the best plan of care for individuals based on their current state of Alzheimer’s and dementia-related disorders.
Walk with Me Strategy
We complete a biography with the resident’s families to gain background knowledge of the resident.
Our practitioners discuss a “day in the life” from current and previous memories to help determine the best programs for individuals.
